2025 CPT code 93565

Left heart catheterization, including left ventriculography, imaging supervision, interpretation, and report, when performed.

Refer to CPT coding guidelines for specific instructions on coding left heart catheterization procedures and appropriate use of add-on codes.

Modifiers may be applicable to 93565 depending on the circumstances of the procedure (e.g., reduced services, unusual non-overlapping services performed by a different provider). Refer to current year's guidelines.

Medical necessity must be established for left heart catheterization. This typically involves demonstrating that the information obtained from the procedure is necessary for diagnosis or treatment planning and cannot be obtained through less invasive means.

The physician introduces a catheter into the left ventricle or left atrium under fluoroscopic guidance. Dye is injected into the chamber to assess heart function. The physician supervises image collection, interprets the images, and creates a report.

In simple words: This procedure involves inserting a thin tube (catheter) into the left side of your heart to take images and assess its function. It's commonly used to diagnose heart conditions. The procedure includes taking X-ray images of the heart's chambers and valves, monitoring these images, interpreting the results, and providing a report.

Left heart catheterization includes catheter placement in a left-sided (systemic) cardiac chamber(s) (left ventricle or left atrium) and includes left ventricular/left atrial angiography, imaging supervision, and interpretation, when performed. For reporting purposes, when the morphologic right ventricle is in a systemic (subaortic) position due to congenital heart disease (e.g., certain cases of transposition of the great arteries), catheter placement with hemodynamic assessment of the subaortic ventricle performed during left heart catheterization is considered part of the procedure and does not constitute right heart catheterization. If additional catheterization of right heart structures (e.g., atrium, pulmonary artery) is performed at the same setting, report the appropriate code for right and left heart catheterization.

Example 1: A patient with suspected mitral valve stenosis undergoes left heart catheterization to evaluate the severity of the narrowing., A patient with a history of myocardial infarction undergoes left heart catheterization to assess left ventricular function and identify any blockages in the coronary arteries., A pediatric patient with congenital heart disease undergoes left heart catheterization to evaluate cardiac anatomy and hemodynamics.

Documentation should include the indication for the procedure, details of catheter placement, imaging findings (including left ventriculography), interpretation of the results, and any complications.
