2025 CPT code 93566

Injection procedure during cardiac catheterization for selective right ventricular or right atrial angiography.

93566 is reported separately in addition to the primary cardiac catheterization procedure code. It is used for selective injection procedures into the right ventricle or right atrium during cardiac catheterization, including imaging supervision, interpretation, and report generation. It is not to be used for non-selective angiography or for angiography of other heart chambers or vessels.

Modifiers may be applicable to 93566 to indicate specific circumstances of the procedure, such as reduced services (modifier 52), distinct procedural service (modifier 59), or other situations as defined by CPT guidelines.

Medical necessity for 93566 is established when selective right ventricular or right atrial angiography is performed during cardiac catheterization to evaluate specific cardiac abnormalities or to guide treatment decisions. The procedure should be performed when other less invasive diagnostic methods are insufficient to provide the necessary information.

The physician inserts a catheter into the right side of the heart, specifically the right ventricle or right atrium, and injects a contrast dye.Under fluoroscopic guidance, images are taken to evaluate the structure and function of these heart chambers. The physician supervises the imaging process, interprets the results, and prepares a report.

In simple words: During this procedure, the doctor inserts a catheter into a vein or artery, usually in the groin or arm, and guides it to the heart. Dye is injected into the right ventricle or right atrium of the heart while x-ray images are taken. This helps the doctor see how the heart is working. This procedure is often done as part of a larger heart evaluation, especially in people with congenital heart disease.

Injection procedure during cardiac catheterization including imaging supervision, interpretation, and report; for selective right ventricular or right atrial angiography (List separately in addition to code for primary procedure). Notes: (Use 93566 in conjunction with 33741, 33745, 93451, 93453, 93456, 93457, 93460, 93461, 93582, 93593, 93594, 93595, 93596, 93597) (Do not report 93566 in conjunction with 33274, 0795T, 0796T, 0797T, 0801T, 0802T, 0803T, 0823T, 0824T, 0825T, for right ventriculography performed during leadless pacemaker insertion) (Do not report 93566 in conjunction with 0545T for right ventricular or right atrial angiography procedures intrinsic to the annulus reconstruction procedure)

Example 1: A patient with a suspected atrial septal defect undergoes cardiac catheterization. Code 93566 is used in conjunction with other catheterization codes to report the selective angiography of the right atrium and ventricle to assess the defect and surrounding structures., A patient with pulmonary hypertension undergoes right heart catheterization. During the procedure, the physician performs selective angiography of the right ventricle to evaluate its size and function. Code 93566 is reported in addition to the right heart catheterization code., A child with congenital heart disease undergoes cardiac catheterization to evaluate cardiac function. Right heart catheterization is performed, and during the procedure, selective angiography of the right ventricle is done to visualize the chamber and assess blood flow. Code 93566 is reported along with other relevant catheterization codes.

Documentation should support the medical necessity of the selective right ventricular or right atrial angiography. The report should include details of the catheterization procedure, the injection of contrast material, and the interpretation of the angiographic images. Any associated diagnoses, such as congenital heart defects or other cardiac conditions, should also be documented.
