Medical necessity is established when the patient requires home INR monitoring due to their prescription for warfarin.This reduces the risk of complications from inadequate anticoagulation or over-anticoagulation.

The physician or qualified healthcare professional is responsible for providing the face-to-face training, ensuring the patient/caregiver understands the procedure, and documenting the training session.

IMPORTANT:May be used with modifier 25 if a separately identifiable E/M service is performed the same day.Should not be reported with 93793 on the same day.Do not report with 98966, 98967, 98968, 98970, 98971, 98972, 99421, 99422, 99423, 99441, 99442, 99443, when telehealth services address home INR monitoring. Time spent should not be counted towards 99439, 99487, 99489, 99490, 99491, if reported in the same month.
