2025 CPT code 95812

Extended EEG monitoring; 41-60 minutes.

Codes 95812-95822 include hyperventilation and/or photic stimulation when appropriate. Routine EEG codes 95816-95822 include 20 to 40 minutes of recording. Extended EEG codes 95812-95813 include reporting times longer than 40 minutes. Recording time excludes setup and takedown time. If diagnostic EEG recording is disrupted, recording time stops until diagnostic EEG recording is resumed.

Modifier 26 (professional component) may be appended if only the professional interpretation is billed. Modifier TC (technical component) may be appended if only the technical component is billed. Modifiers should not be added when billing for both professional and technical components.

Medical necessity for an extended EEG must be established based on clinical findings suggesting the need for a prolonged recording period to capture intermittent or subtle neurological events not detectable during shorter studies. This is often the case for patients with suspected epilepsy, sleep disorders, or other conditions that present with periodic or paroxysmal activity.

The physician or qualified healthcare professional is responsible for placing electrodes, recording brainwave activity, interpreting the results, and generating a report. This may involve additional tasks such as patient preparation, hyperventilation, and photic stimulation if clinically indicated.

IMPORTANT:Do not report 95812 in conjunction with 95700-95726.

In simple words: This code describes a longer-than-usual brainwave test (EEG) that lasts between 41 and 60 minutes. The test uses small sensors on the head to check for unusual brain activity, helping doctors diagnose conditions like epilepsy or sleep problems.

This CPT code, 95812, represents an extended electroencephalography (EEG) monitoring procedure lasting between 41 and 60 minutes. It involves the application of electrodes to the scalp to record and analyze brain electrical activity. This extended duration is used for comprehensive evaluation of neurological conditions such as epilepsy, sleep disorders, or other suspected brain disorders requiring prolonged monitoring to capture intermittent or subtle abnormalities.

Example 1: A patient presents with suspected epilepsy experiencing infrequent seizures. An extended EEG is performed to capture potential seizure activity that may not occur during a shorter recording., A patient is admitted to the hospital in a coma of unclear etiology. An extended EEG is ordered to monitor for periodic changes that might provide insight into the cause and severity of the coma., A patient complains of recurrent sleep disturbances and daytime sleepiness. A prolonged EEG study, including sleep stages, can help differentiate between various sleep disorders and identify possible underlying neurological issues.

Detailed patient history including symptoms and duration, reason for the extended EEG, results of the EEG recording, physician's interpretation, and the diagnosis.
