2025 CPT code 96547

Intraoperative hyperthermic intraperitoneal chemotherapy (HIPEC) procedure, including separate incision(s) and closure, when performed; first 60 minutes.

This is an add-on code and should be reported in addition to the primary procedure. Report 96548 for each additional 30 minutes.

Modifiers may be applicable, such as 52 (Reduced Services), 53 (Discontinued Procedure), or others, based on the circumstances of the procedure.

HIPEC is medically necessary for treating certain cancers that have spread to the peritoneum.The medical necessity should be documented in the patient's medical record and linked to the primary surgical procedure.

The physician or other qualified healthcare professional is responsible for selecting the chemotherapy agent, confirming perfusion equipment settings, supervising the perfusion and manual agitation of the chemotherapy agent, irrigating the abdominal cavity, closing the incisions, and documenting the procedure.

IMPORTANT:Use 96548 for each additional 30 minutes of HIPEC.

It does not include work related to the primary procedure.

Example 1: A patient undergoing cytoreductive surgery for peritoneal carcinomatosis receives HIPEC with cisplatin for 60 minutes following tumor removal., A patient with appendiceal cancer undergoes surgery and receives HIPEC with mitomycin C for the first hour after resection., A patient with mesothelioma undergoes a pleurectomy/decortication and receives HIPEC for 60 minutes.

Documentation should include the chemotherapy agent used, the perfusion time, details of the HIPEC procedure (including incisions, catheter placement, perfusion parameters, and agitation technique), irrigation details, wound closure information, and any complications.
