Initial comprehensive preventive medicine evaluation and management of a child, new patient (age 1 through 4 years).

If a significant, separately identifiable problem is encountered during the preventive visit that requires additional work, the appropriate problem-oriented E/M code (e.g., 99212-99215) should also be reported with modifier 25 appended. Modifier 33 may be used to indicate preventive services.
