Initial comprehensive preventive medicine evaluation and management of an individual including an age and gender appropriate history, examination, counseling/anticipatory guidance/risk factor reduction interventions, and the ordering of appropriate laboratory/diagnostic procedures, new patient; late childhood (age 5 through 11 years).

If an abnormality is found or a pre-existing condition is addressed during the preventive visit and requires additional work, an appropriate office/outpatient E/M code should be reported in addition to 99383, with modifier 25 appended to the E/M code.Vaccine/toxoid products, immunization administrations, and other procedures/tests are reported separately.

Modifier 25 may be applicable if a significant, separately identifiable E/M service is provided on the same day.

The service is medically necessary for the preventive healthcare of a child within the specified age range who is a new patient to the practice.

The physician is responsible for performing an age and gender appropriate history and physical examination, providing counseling/anticipatory guidance/risk factor reduction interventions, and ordering appropriate immunizations/laboratory/diagnostic procedures based on the patient’s risk factors.
