Initial comprehensive preventive medicine evaluation and management for a new patient aged 65 or older.

Modifier -25 may be added if a significant, separately identifiable E/M service is performed on the same day. Modifier -33 (Preventive Services) may be used.

Medical necessity is established when the patient is a new patient aged 65 or older and requires a comprehensive preventive medicine evaluation and management service. This includes age and gender-appropriate screenings, counseling, and risk factor reduction.The service must be medically appropriate and necessary for the individual's health needs.

The physician's clinical responsibility includes performing a comprehensive history, physical examination, counseling, and ordering of any necessary tests or immunizations. This includes assessing the patient's risk factors, providing anticipatory guidance, and developing a personalized preventive care plan.

IMPORTANT:Codes 99381-99386 cover preventive medicine services for new patients of other age groups.Codes 99391-99397 cover established patients.Modifier 25 may be used if a significant, separately identifiable E/M service is performed on the same day.

The physician performs a complete history, physical exam, including age-appropriate screenings (e.g., prostate exam, colon cancer screening discussion), orders blood work and influenza vaccination. Code 99387 is reported., A 68-year-old female, new to the practice, comes in for an annual well-woman exam.The visit includes a comprehensive review of systems, a breast exam, pelvic exam, and discussion of preventative measures for osteoporosis and heart disease.Pap smear and mammogram are ordered. Code 99387 is reported.Additional codes for the Pap smear and mammogram will be reported separately., A 65-year-old male new patient presents for his first visit with complaints of mild fatigue and joint pain. The doctor performs a thorough history and physical exam, including addressing the fatigue and joint pain complaints. Because these complaints required additional work beyond the preventive visit the provider also reports an additional E/M code (e.g., 99203) with modifier -25.
