Follow current CPT coding guidelines for preventive medicine services.Ensure that the service provided is distinct from any E/M services rendered on the same day.

Modifier 25 may be appended if a significant, separately identifiable E/M service is performed on the same day.

IMPORTANT:Codes 99401-99403 represent shorter counseling sessions (15, 30, and 45 minutes respectively).If significant, separately identifiable E/M services are provided on the same day, use the appropriate E/M code with modifier 25.
