Drug test(s), definitive, using methods to identify individual drugs and distinguish between structural isomers (but not stereoisomers), including GC/MS and LC/MS, excluding immunoassays.

Do not report G0659 with other definitive drug testing codes for a specific number of drug classes on the same date of service. When billed with a presumptive drug test (80307) on the same date of service, only G0659 may be reimbursed. Modifier 90 should be used if the test is performed by a reference laboratory.

Modifiers may be applicable. For example, modifier 90 is used when the test is performed by an outside laboratory.

Medical necessity must be established for this test. This typically involves documentation of symptoms, signs, or circumstances that warrant the test, such as overdose, substance abuse treatment, or workplace drug testing requirements.

IMPORTANT:Other definitive drug testing codes (G0480-G0483) are available for specific numbers of drug classes.Presumptive drug tests (80305-80307) are also available.
