Remote assessment of recorded video and/or images submitted by an established patient, including interpretation and follow-up within 24 business hours.

This code should only be reported when the service is unrelated to a previous service within the past seven days, and no related service or procedure results from this service within the next 24 hours or at the next available appointment.

This code is intended for use when an in-person visit is not feasible and the patient's condition can be adequately assessed through remote monitoring of recorded video and/or images.The medical necessity must be documented in the patient's medical record.

A provider who cannot independently bill for evaluation and management (E/M) receives recorded video and/or images remotely from a patient. They assess the material, analyze it, interpret the findings, and follow up with the patient within 24 business hours.

In simple words: A healthcare provider reviews videos or images sent by a patient, interprets the findings, and follows up with the patient within one business day. This is used when the patient cannot have an in-person appointment and hasn't had a related service recently.

Remote assessment of recorded video and/or images submitted by an established patient (e.g., store and forward), including interpretation with follow-up with the patient within 24 business hours, not originating from a related service provided within the previous 7 days nor leading to a service or procedure within the next 24 hours or soonest available appointment.

Example 1: A patient sends a video of a rash to their dermatologist for evaluation. The dermatologist reviews the video, makes a diagnosis, and contacts the patient within 24 business hours with treatment recommendations., A patient with a chronic wound sends images of the wound to their wound care specialist for monitoring. The specialist reviews the images, assesses the healing progress, and provides feedback to the patient within 24 business hours., A patient sends a video of their child's gait to their physical therapist for evaluation. The therapist reviews the video, identifies any gait abnormalities, and follows up with the patient within 24 business hours to discuss treatment options.

The documentation should include:* The date and time the video/images were received.* The type of video/images submitted (e.g., video of rash, images of wound).* The provider's interpretation of the findings.* The plan for follow-up with the patient.* Confirmation that the service was unrelated to any service provided in the last 7 days and didn’t lead to a service or procedure within 24 hours or the soonest appointment.
