Virtual check-in service by a qualified healthcare professional for an established patient, involving 11-20 minutes of medical discussion.

This code should not be reported for services that are part of a bundled service or that result in a subsequent E/M service or procedure within 24 hours.

Modifiers may be applicable, such as those related to telehealth or location. Modifier FQ should be used for audio-only telehealth visits.

The medical necessity should be documented by the reason for the virtual check-in and why a face-to-face appointment wasn't immediately necessary.

The provider spends 11–20 minutes in technology-based communication with an established patient. The provider must be a physician or other healthcare professional who is qualified to report an E/M service.This service cannot be related to a previous E/M service within the past seven days, nor can it lead to a related E/M service or procedure within the next 24 hours or at the next available appointment.

IMPORTANT:Crosswalked to CPT code 99442 for reimbursement purposes.

Brief communication technology-based service, e.g., virtual check-in, by a physician or other qualified healthcare professional who can report evaluation and management services, provided to an established patient, not originating from a related E/M service provided within the previous 7 days nor leading to an E/M service or procedure within the next 24 hours or soonest available appointment; 11-20 minutes of medical discussion.

Example 1: An established patient with asthma calls their physician using a telehealth platform to discuss worsening symptoms. The physician spends 15 minutes discussing the symptoms, medication adjustments, and the need for an in-person follow-up., A patient with diabetes has a virtual check-in with their nurse practitioner to discuss recent blood glucose levels. They spend 12 minutes reviewing the data and adjusting insulin dosage., A patient recovering from surgery has a 18-minute telehealth call with their physician to discuss post-operative pain management. They discuss pain levels, medication side effects, and wound care.

Documentation should include the total time spent in medical discussion (11-20 minutes), the content of the discussion, and that the service meets the criteria for G2252 (not related to a recent or upcoming E/M service).
