This service was performed in whole or in part by a resident in a Department of Veterans Affairs medical center or clinic, supervised in accordance with VA policy.

Use modifier GR for services provided by residents in VA facilities. Do not use modifiers GC or GE on VA claims, even if they represent similar scenarios. Ensure the claim is submitted under the attending provider's name.

Medical necessity should be supported by appropriate documentation as it would be for any other claim, even with a resident involved.

The resident provider performs the service under the supervision of the attending provider.

IMPORTANT:Has similar implications to modifiers GC (This service has been performed in part by a resident under the direction of a teaching physician) and GE (This service has been performed by a resident without the presence of a teaching physician under the primary care exception), but GR is used specifically for VA claims.

Modifier GR denotes services rendered in a VA facility by a resident provider, a medical graduate working under the supervision of an attending provider. The VA facility appends modifier GR to the appropriate HCPCS code for the service and submits the claim under the name of the attending provider. This modifier has the same consequences as modifiers GC and GE, but is used specifically for VA claims.

The modifier GR is appended to the appropriate HCPCS code., A resident assists in surgery performed by an attending surgeon at a VA hospital. Modifier GR is appended to the surgical procedure code., A resident provides follow-up care to a veteran patient after a procedure, under the guidance of an attending physician, in a VA medical center. The appropriate HCPCS code for the follow-up care is appended with modifier GR.

Documentation should clearly identify the attending provider and the resident provider involved in the service, along with details about the level of supervision provided.
