2025 ICD-10-CM code I01.0

Acute rheumatic pericarditis.

Code I01.0 should be used when there is documented evidence of acute rheumatic pericarditis. If chronic rheumatic heart disease is also present, code I09.8 should be used as the primary diagnosis.

Medical necessity for the diagnosis of acute rheumatic pericarditis is established by the presence of characteristic clinical signs and symptoms, along with evidence of a preceding streptococcal infection and supporting diagnostic findings.

In simple words: Inflammation of the pericardium (the sac surrounding the heart) due to acute rheumatic fever.

Acute rheumatic pericarditis. Any condition in I00 with pericarditis. Rheumatic pericarditis (acute).

Example 1: A patient presents with chest pain, fever, and friction rub on auscultation. After a thorough evaluation, including echocardiography and blood tests, the diagnosis of acute rheumatic pericarditis is confirmed. The patient has a history of recent streptococcal infection., A child with a history of acute rheumatic fever develops chest pain and shortness of breath. Examination reveals muffled heart sounds and an enlarged cardiac silhouette on chest X-ray. The patient is diagnosed with acute rheumatic pericarditis with effusion., A young adult presents with fever, joint pain, and a new heart murmur. Echocardiography shows evidence of valvulitis and pericardial effusion. Based on the clinical picture and supporting laboratory findings, a diagnosis of acute rheumatic fever with pericarditis is established.

Documentation should include evidence of a preceding streptococcal infection, clinical findings such as chest pain, friction rub, and muffled heart sounds, as well as supporting laboratory and imaging studies like echocardiography.

** Excludes1: acute pericarditis not specified as rheumatic (I30.-)
