Hydronephrosis with renal and ureteral calculous obstruction

Do not use this code if there is an associated infection; instead use N13.6 (Pyonephrosis). This code excludes congenital obstructive defects of the renal pelvis and ureter (Q62.0-Q62.3) and calculus of kidney and ureter without hydronephrosis (N20.-).

Medical necessity is established by the presence of hydronephrosis along with the identification of an obstructing renal or ureteral calculus, causing symptoms or posing a risk of complications if left untreated.

In simple words: Swelling of the kidney due to a blockage caused by a kidney stone in the tube connecting kidney to bladder.

Hydronephrosis occurs when urine backs up into the kidney, causing it to swell. This condition is caused by a blockage in the ureter, the tube that carries urine from the kidney to the bladder. In this case, the blockage is due to a calculus (stone) present in the kidney and ureter.

Example 1: A patient presents with flank pain and is found to have hydronephrosis on ultrasound, with a calculus identified in the ureter., A patient with a history of recurrent urinary tract infections now has hydronephrosis with stones discovered on CT scan in the kidney and ureter., During a follow-up visit for known kidney stones, a patient shows evidence of new hydronephrosis, suggesting the stone hasn't passed and is causing obstruction.

Documentation should include imaging evidence of hydronephrosis (such as ultrasound or CT scan) and the presence of a calculus in the kidney and/or ureter, along with clinical symptoms like flank pain or changes in urination.
