Code N46.025 should be used when the azoospermia is due to a documented systemic disease. The underlying systemic disease should also be coded.

Medical necessity for the evaluation and treatment of azoospermia is established by the patient's desire for fertility and the presence of azoospermia confirmed by semen analysis.

Diagnosis and treatment by a urologist or other specialist in male reproductive health.

In simple words: The inability to produce sperm due to a disease affecting the whole body.

Absence of sperm in the semen caused by a systemic disease.

Example 1: A 35-year-old male presents with primary infertility and is found to have azoospermia. Further workup reveals a diagnosis of diabetes, a systemic disease, which is deemed the cause of his azoospermia., A patient with a history of kidney failure (a systemic disease) is evaluated for infertility and is found to have azoospermia., A patient undergoing chemotherapy for lymphoma (a systemic disease) experiences azoospermia as a side effect of the treatment.

Documentation should include medical history, physical exam findings, semen analysis results, and any other diagnostic tests performed to identify the underlying systemic disease causing the azoospermia.
