Infection of obstetric surgical wound. This includes infected cesarean delivery wound and infected perineal repair following delivery.

Code O86.0 is a valid code for 2024, but more specific codes (O86.00-O86.09) are available. Code O86.0 should only be used on the maternal record, never on the newborn record. The appropriate code for weeks of gestation (Z3A.-) should be used as a secondary code.

Medical necessity is established by the presence of signs and symptoms of infection at an obstetric surgical wound. Treatment is necessary to prevent complications such as sepsis or further spread of infection.

Obstetricians and other healthcare professionals providing postpartum care are responsible for diagnosing and managing infections of obstetric surgical wounds.

Example 1: A patient develops an infection at her cesarean section incision site one week postpartum., A patient experiences pain, redness, and swelling at her episiotomy repair site three days postpartum, and an infection is diagnosed., A patient undergoes a cerclage placement during pregnancy, and the surgical site becomes infected two weeks later.

Documentation should include the location of the wound infection (e.g., cesarean incision, perineal repair), signs and symptoms of infection (e.g., redness, swelling, pain, purulent drainage), and any cultures taken to identify the causative organism. If a specific organism is identified, it should be coded with a B95-B97 code in addition to O86.0.
