2025 HCPCS code Q0084

Chemotherapy administration by infusion technique only, per visit.

Refer to payer guidelines to determine which codes are accepted for chemotherapy administration.

Medical necessity for chemotherapy administration should be supported by documentation of the patient's cancer diagnosis, treatment plan, and expected benefits of chemotherapy.

The provider sterilizes the insertion area, inserts the needle into a blood vessel, intravenous line, subcutaneous catheter, or port, infuses the chemotherapy medication, and monitors the patient’s reaction.

IMPORTANT For chemotherapy administration other than infusion, use Q0083. For chemotherapy given by both infusion and other methods, use Q0085. For infusion therapy using non-chemotherapeutic drugs, use Q0081. CPT codes 96401-96549 also represent chemotherapy administration.

In simple words: This code covers giving chemotherapy medicine to a patient through an IV drip, not through a shot or other ways.

This code represents the administration of chemotherapy drugs using an infusion technique, excluding other administration methods like subcutaneous, intramuscular, or intravenous push. It is reported once per visit.

Example 1: A patient receives chemotherapy through a central line., Chemotherapy is administered via a peripherally inserted central catheter (PICC line)., A patient receives chemotherapy through an implantable port.

Documentation should include the type of infusion device used, the chemotherapy drug administered, the dosage, the duration of the infusion, and the patient's response to the treatment.
