2025 HCPCS code S2325

Hip core decompression for the treatment of osteonecrosis or avascular necrosis of the femoral head.

This code should only be used for early-stage (pre-collapse) AVN or osteonecrosis of the femoral head.It is not payable by Medicare. Some private payers may require this code for reimbursement.

Modifiers may be applicable to further specify the circumstances of the procedure.

Medical necessity should be established by documenting the diagnosis of early-stage AVN or osteonecrosis, the patient's symptoms, and the rationale for performing core decompression. This rationale should include the intent to prevent femoral head collapse and subsequent arthritis. The documentation should support that less invasive treatments have been considered or tried and were unsuccessful.

The physician prepares and anesthetizes the patient, performs the core decompression procedure, and may perform bone grafting. They also close and dress the surgical wound.

In simple words: This procedure helps to relieve pressure and improve blood flow to the hip bone in the early stages of bone death due to poor blood supply. The surgeon drills small holes in the hip bone, sometimes adding a bone graft to help healing. This helps prevent the hip bone from collapsing and developing arthritis.

This code represents core decompression of the femoral head (hip) to relieve pressure and encourage new blood vessel formation. It is typically used in the early stages (pre-collapse stage I or II) of avascular necrosis (AVN) or osteonecrosis of the femoral head. The procedure may include drilling one large hole or several small holes into the femoral head. Bone grafting may be performed following the decompression.

Example 1: A 45-year-old patient presents with early-stage avascular necrosis of the femoral head. S2325 is used to report the core decompression procedure performed to prevent further bone damage., A 50-year-old patient with pre-collapse stage II AVN of the femoral head undergoes core decompression with bone grafting. S2325 is reported for the decompression procedure., A patient with osteonecrosis of the femoral head is treated with core decompression.S2325 is billed by the provider.

Documentation should include the diagnosis of AVN or osteonecrosis, the stage of the disease, a description of the procedure performed (including the number and size of holes drilled and whether bone grafting was performed), and any associated complications.

** This code is a temporary national code and is not recognized by Medicare. Use of this code may be required by some private payers. If the procedure involves additional procedures, such as bone grafting, separate codes may be required to report these services.
