2025 ICD-10-CM code T14.90

Injury, unspecified. This code is used when the specific location or nature of the injury is not documented.

Use additional code to identify any retained foreign body, if applicable (Z18.-). Do not use this code for birth trauma (P10-P15) or obstetric trauma (O70-O71).

Medical necessity must be established for any injuries documented. The documentation should clearly support the need for medical care related to the injury.

IMPORTANT:T14.90 has been replaced by T14.90xA (initial encounter), T14.90xD (subsequent encounter), and T14.90xS (sequela).

In simple words: This code is used when a person has an injury, but the medical records don't specify what kind of injury it is or where it is located on the body.

Injury, unspecified. This code is used when the documentation does not specify the location or type of injury, and a more specific code is not available. Use secondary code(s) from Chapter 20, External causes of morbidity (V01-Y99), to indicate cause of injury.

Example 1: A patient presents to the emergency room after a motor vehicle accident with generalized pain but no specific injuries documented., A patient reports falling, but the documentation doesn't specify the location or nature of any injuries sustained., A patient has an unspecified injury documented in their medical chart without further detail.

Detailed documentation is required specifying the type of injury and location to use a more specific code. If the location and type are unspecified, then T14.90 is appropriate until more information is available. Document the cause of injury using a code from Chapter 20, External causes of morbidity (V01-Y99).

** As of October 1, 2017, code T14.90 was replaced with more specific codes for initial and subsequent encounters and sequela. Always verify the current coding guidelines.
