Both Z94.1 (Heart transplant status) and the appropriate code for pneumonia are reported., A patient who received a liver transplant is seen for management of their immunosuppressant medication. The code Z94.4 (Liver transplant status) is used.

Documentation should specify the organ or tissue transplanted, the date of the transplant, and any complications or related conditions.
