2025 CPT code 81379

HLA Class I typing, high resolution (ie, alleles or allele groups); complete (ie, HLA-A, -B, and -C)

This code should be used for high-resolution typing of HLA Class I loci A, B, and C. For typing at lower resolution or other HLA loci, different codes should be used.

Modifier 26 can be used if only the professional component (interpretation and report) is performed by the physician.

Medical necessity for this test must be established based on the patient's clinical condition. Justification for HLA typing includes organ or bone marrow transplantation, diagnosis of certain diseases associated with specific HLA types, or other clinically relevant indications.

The laboratory analyst is responsible for performing all the technical aspects of the procedure. This includes nucleic acid extraction, amplification, and detection to identify the HLA alleles.

In simple words: This test analyzes specific genes to determine the type of HLA proteins present in your cells. These proteins help your immune system distinguish between your own cells and foreign substances. This high-resolution test examines the HLA-A, -B, and -C genes, providing detailed information about your specific HLA type.

This code represents high-resolution typing of Human Leukocyte Antigen (HLA) Class I loci A, B, and C using molecular techniques. It includes all analytical steps involved in the process, starting from cell lysis to identifying the specific alleles or allele groups.

Example 1: A patient requires a kidney transplant. HLA typing is performed at high resolution for both the donor and recipient to ensure the best possible match and minimize the risk of organ rejection., A patient presents with symptoms suggestive of an autoimmune disorder. High-resolution HLA typing is ordered to identify specific HLA alleles associated with certain autoimmune diseases, aiding in diagnosis., A patient with leukemia requires a bone marrow transplant. HLA typing of Class I loci is performed on potential donors to assess their compatibility with the recipient and ensure a successful transplant.

Documentation should include the patient's medical history, reason for testing (e.g., transplant, autoimmune disease diagnosis), and the specific HLA loci tested. The report should list the identified alleles or allele groups.
